COVID 19 - Appointments proceedure and information

A 48 hour cancellation notice applies to bookings, however, allowance is made for changes due to any illnesses, or Covid symptoms or potential exposure.
If you need to change a face to face booking to a phone or video session, any resulting overpayment in fee balance will be refunded to you where relevant. You can have your reading by phone or video call for the same appointment time if you prearrange this well in advance.

Social Distancing in one to one in person bookings
Social distancing is practised throughout in person appointments.
In person appointments are considered to be for one person attending.
If you require someone to come into your appointment with you and sit with you during your booking, you will need to request this at least 48 hours or 2 business days in advance, whichever is larger, as a larger office will have to be booked and hired to accommodate this.
Masks
Masks are not used in video call appointments.
Masks are still used for in person one to one bookings.
You will be required to wear a mask to and during your in person appointment.
If you cannot wear a mask due to medical reasons, please consider changing your appointment to video or phone appointment.
Jacquelene is asthmatic and has lowered immunity, and will be wearing a mask during all in person appointments.
For in person appointments, you are therefore asked to consider everyone's health and safety when booking and that you be truthful about your situation, contacts, and symptoms.
URGENT TO NOTE ; WHEN AND WHY TO CHANGE YOUR BOOKING
Please immediately change your face to face booking to a phone or video appointment or reschedule your appointment if you have :
If any of these symptoms or circumstances apply to you now or over the last 10 days, or if you have a suppressed immune system please either take your booking as a phone or video session or reconvene your booking by text 0439488558 to when you are well and are given medical clearance.

- Been in contact with anyone with COVID 19 or in contact with a contact of a person with COVID 19, in the last 10 days,
- Whether you have in the last 10 days had ANY covid symptoms or fever, dry cough, cold , hay fever, or flu like symptoms, or loss of smell or taste.
- You have been required by health orders to isolate, or you have tested positive to Covid 19.
If any of these symptoms or circumstances apply to you now or over the last 10 days, or if you have a suppressed immune system please either take your booking as a phone or video session or reconvene your booking by text 0439488558 to when you are well and are given medical clearance.
COVID-19 Liability Waiver
By going ahead with an in person appointment with Jacquelene Close Moore, You agree to this waiver.
- That I acknowledge the contagious nature of the Coronavirus/Covid -19 and that many public health authorities recommend practicing social distancing, mask wearing, Covid - 19 Vaccinations, and other health measures. I further acknowledge that Jacquelene Close Moore has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, and is acting within the requirements of the government policies as relevant at the time of my booking being made, and at the date of my appointment.
- I further acknowledge that Jacquelene Close Moore cannot guarantee that I will not become infected with the Coronavirus/Covid-19.
- I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Jacquelene Close Moore, staff, other clients, and their families.
- I voluntarily seek services provided by Jacquelene Close Moore, and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19 by attending an in person consultation. I agree to show my Photo ID, and my vaccine status upon check in, (green tick on the check in Victorian Government check in app) or I agree to show paper copy of my vaccination or exemption certificate as well as photographic proof of identity upon arrival to my booking. I accept that Jacquelene will not store any details of the documents I provide, but that Jacquelene will safely store a copy of this signed waiver.
- I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
- I attest that:
I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell and have not had any of these symptoms in the last 10 days.
I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19 in the last 10 days.
I have not been diagnosed with Coronavirus/Covid-19 in the last 10 days, or if having in the past been diagnosed with COVID-19 that I have been cleared as non contagious by state or local public health authorities more than 10 days ago.
I am following all government recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. - I have made a truthful declaration of my circumstances relating to all points on this waiver, and hereby release and agree to hold Jacquelene Close Moore harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Jacquelene Close Moore or staff, or that may otherwise arise in any way in connection with any services received from Jacquelene Close Moore. I understand that this release discharges Jacquelene Close Moore from any liability or claim that I, my heirs, or any personal representatives may have against her or staff with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Jacquelene Close Moore. This liability waiver and release extends to the Jacquelene Close Moore together with all partners, and employees.
